A complete ophthalmic evaluation often requires in-clinic testing as well as a thorough review of medical records and patient interview followed by a comprehensive eye examination, which can be time-consuming. Depending on your provider and type of appointment, you may need to allow 2 to 4 hours for your first visit. Our team can provide you with more detailed information.

The Cizik Eye Clinic is located in the Texas Medical Center. The clinic is on the 18th floor of the Memorial Hermann Medical Plaza, located at 6400 Fannin Street.
Convenient self-parking is available in the building’s parking garage, which is accessible from both Fannin Street and Main Street. The patient drop off lane is also accessible by turning right into the first entrance on Fannin Street.
Valet parking is also available. To utilize valet parking, turn right into the second entrance on Fannin Street. You can also enter the valet area from Main street, by turning at the Hermann Plaza Drive streetlight.
Once you park, ride the elevator in the parking garage to the first floor lobby. Then, ride to the 18th floor, using the elevator bank that services floors 14-24.

You should have received an invitation to sign up for the MyUTHealth Patient Portal at the email address you provided during the time of registration. This secure site is where you will input your personal information and your medical history and sign consent forms, so our clinicians can provide you with the best care possible.
All patients can complete the required forms by logging in to the Patient Portal before their appointment.
If you did not receive an invitation to sign up for the MyUTHealth Patient Portal, please ensure you are using the email address that you provided upon registration. The invitation might filter to your junk mail, so please be sure to check your spam/junk folders if you do not receive it.
We also request that you bring any medical records you may have to your appointment, along with your insurance card(s) and photo ID.
